Exclusively for individuals recognized as Special Services members. Special Services members must submit the form to apply discount eligibility to their Special Services Discount online account. The form is NOT to be passed on to anybody outside of the eligible Special Services Associations.

Usage Conditions:
•    Discount is for the Special Services personal use only.
•    No minimum spend required.
•    Excludes items already on sale.
•    Cannot be used with other discounts or promotions.

Verification:
Mizuno may request proof of Special Services status at any time and retains the right to suspend access in cases of misuse or misrepresentation.
